Hounslow Synergy Provider Portal – Early Years User Form

This form must be completed by either a Manager, Deputy Manager or Childminder.

Type of request that can be made:
 

• Request a login to the Provider Portal to 2, 3 and 4 year old early years funding, childcare provider form and training
• Change of mobile phone number
• Change in email address
• Change in security level to 2FA (2 factor authentication)*
• Close Account

*User will need a smart phone to download an app for 2 factor authentication
