Subject Access Requests

Please use this form to make a Subject Access Request for personal information held by the London Borough of Hounslow.

A Subject Access Request (SAR) is simply a written request made by or on behalf of an individual for the information which he or she is entitled to ask for under section 7 of the Data Protection Act 1998 (DPA).

To enable the council to respond quickly to the request, we ask that proof of identification is uploaded as part of the online form process. We require two forms of evidence of your identity. One form must be photo evidence; either a current driving licence or a current passport. One form must be proof of your address; either a utility bill (excluding mobile phone bill), bank statement, or council tax bill. It will help if you have scanned or photographed images, ideally in colour, of these identification documents ready before beginning the form. Please note that we cannot proceed with the request unless satisfactory identification is provided.

If you are completing the form on behalf of someone else you may also need a scanned or photographed image of a document signed by them giving their consent for you to make the request.
