Completing an assessment
Whether you are a carer or the person who is ‘cared for’, we can provide you with information and guidance on needs assessments.
Online assessment
The assessment form can be completed in as little as 10 minutes. You can save it online and you have up to 30 days to submit it directly to the council, safely and securely. A member of the First Contact Team will then contact you within five working days.
Telephone assessment
A carers assessment can also be completed by telephone by contacting the First Contact Team on 020 8583 3100 or by email at adultsocialcare@hounslow.gov.uk.
Help with assessment
If you have difficulty accessing or understanding the process, you are entitled to an independent advocate to support you and represent your point of view. To request an advocate, contact the First Contact Team.
