A new self-referral solution for adult social care is being introduced to make it easier for residents to receive the right support, at the right time, from the right team.
Residents who create a portal account will be able to view updates on their enquiries’ journey with early signposting to other Council teams enabling the streamlining of urgent adult social care issues.
This new, more efficient process includes built-in filtering, and residents can self-refer online or by phone, receiving clear updates at each stage.
Commenting on the new development, Hounslow Council’s Cabinet Member for Health, Wellbeing and Adult Social Care, Councillor Lily Bath said:
“The new self-referral process will provide residents with a clearer route through their adult social care journey. It’s more person-centred and accessible, ensuring improved transparency of information among relevant Council teams. Demand for adult social care continues to rise so increased efficiencies like this are important.”
