Payment can only be made online using a debit or credit card.
Your card provider may send a text message to the mobile number associated with this card for verification. Please make sure you have your mobile with you.
On the payment screen:
Please enter the name exactly as it appears on the credit or debit card you are using for this transaction.
Please make sure that you also enter the HMO address in the address fields.
Please note:
Once you click "PROCEED TO ONLINE PAYMENT" you will not be able to retrieve this form if you have saved it
Please make sure that you are in a position to to make the payment, otherwise you will have to complete the form again at a later time.
