If you need help to live independently in the community, Hounslow Council has organised its adult social care and well being services to ensure that you are in charge of the support you need.
Putting People First in Hounslow will give you the choice and control to shape your life. You will be supported to decide what services you want, where they take place and who will provide them. We will support you to access universal services such as transport, leisure and libraries as well as services that help you to remain or become more independent.
Regardless of your financial situation, or your level of need, you will be able to get hold of accurate, reliable,up-to-date information and support, not only about council services, but services provided across the borough.
If you have a high level of support needs you may be eligible for your own personal budget from the council. This will help you to plan and buy the support/services you need. With the help of a council care manager, family, friends or an independent broker, you can put together your own support plan.
If you already have a care manager, they will guide you through this process. If you do not have a care manager and would like to ask for an assessment, please contact us by emailing adultsocialcare@hounslow.gov.uk or calling 020 8583 3100.
