In-house assessment and reablement service

If you are recovering from illness or have been in hospital, it may take some time to get back in your normal routine.

The in-house assessment and reablement service can provide practical support and help with your personal care needs and other daily activities. The service normally lasts for a short period of time and can help develop your skills and confidence so you can function safely and independently at home.

The in-house assessment and reablement team aim to:

  • prevent admissions into hospital;

  • reduce the time of hospital stays;

  • help ensure long-term residential care isn’t needed; and

  • provide support to maintain or regain independence.

Who is this service for?

This service is for all Hounslow residents aged 18 and above, who are struggling at home following an illness or hospital stay. This includes people who are:

  • physically frail;

  • disabled; or

  • recovering from recent illness or injury.

You will need to be assessed to find out if you are eligible to get this service.

Will I have to pay?

The service is free but you will need to be assessed to see if you are eligible.

How do I get this service?

You must be referred by the Adult Access Team to get this service.

The Assessment and Reablement Team is based at the Civic Centre and covers all areas of the borough, split into East and West districts.

When is the service available?

The Reablement and Assessment Service is available from 7am – 10.30pm, 365 days of the year.

We also operate an Emergency Responder Service for people who need support outside of these hours.

Emergency Responder Service (ERS)

The Home Care Emergency Responder Service (ERS) provides emergency cover from 5pm-9am, Monday to Friday and continuously during weekends and Bank Holidays to subscribers of LinkLine or LinkLine+ who sign up to this additional service.

It aims to support residents wishing to remain independent within their own homes and can offer respite care for family and friends by acting as a key holder for emergency call out visits.

Carers emergency card scheme

The carers emergency card scheme is designed give carers peace of mind knowing that the person they care for will continue to be cared for in case of an emergency.

The carers emergency card is a credit card-sized card which contains a personal identification number, unique to each carer. Carers should keep this card with them so it can be used as an instant source of identification in an emergency.

Calling the telephone number on the card will trigger a link to a database and begin a pre-agreed process to ensure the cared for person gets the attention they need.
