Common Assessment Framework

Information for parents and carers

If your child needs additional support and you are concerned about their welfare, we want to be able to provide help and assistance as soon as possible.

To enable us to provide this additional support we need to complete a common assessment or CAF.

This involves asking you and your child some questions, to find out what help and support your child might need.

If you agree, a worker you feel comfortable with, like your child’s class teacher or health visitor, will sit down with you and your child and complete the CAF form. You will be asked to sign the form and agree to us sharing the information with other practitioners in order to provide the best possible help and support. A copy of the CAF form will be given to you on completion. Older children may feel able to discuss their situation on their own with the worker.

Based on the information you and your child have provided, a small team of professionals will be set up to work with you and your child and provide the help and support you have identified.
