Support for children in and after care

Futures, the Leaving Care Team, works with 16 to 21 year olds (up to 24 if in Higher Education) who are or have been in care to Hounslow. Each young person receives a Needs Assessment and a Pathway Plan, which records the young persons aspirations and identifies the goals they wish to achieve, and is regularly reviewed.

The Team aims to ensure a positive transition for young people into adulthood, and offers support, advice and guidance on a range of issues affecting young people. We have specialist knowledge and can advise on benefits, welfare rights, accommodation, health, sexuality and many other issues.

Futures also works closely with other departments and organisations in order to access specialist services, e.g. Housing, Connexions, Education, Health etc.

Futures is open from:

  • Monday to Friday 9.00 to 12.30 and 1.30 to 5.00 (4.45 on Fridays),

  • a drop in duty service is available to young people each day (except Wednesdays) from 10.00 to 12.30 and 1.30 to 4.00.
