Carers' information and support packs have been designed to give you basic information about the kinds of services that are available to you.

The packs are regularly updated with new information and are given to you at the time of your carer’s assessment. They were created in partnership with local carers and carers' organisations.

A social care practitioner will go though the information pack with you to make sure you understand the information given and know where to go for help.

Carers support packs are available for carers supporting:

• older people

• people with a physical disability & sensory impairment

• people with a learning disability

• people with a mental health problem
