Hounslow Council

We are trained to help with issues or requests for information relating to your social care needs.

We provide a confidential information and assessment service, whether you are enquiring about help at home following loss of independence, day care for someone or a carer’s assessment for yourself.

To help us understand what help you need, we will ask you a number of questions:

  • about your health

  • your family circumstances

  • what you can and cannot do at home; and

  • what help you think you may need.

The information you provide will help us to decide whether you may be eligible for our services.

If you're not eligible, we will provide you with information about other services that may be able to help you.

If we think you may be eligible, we will ask you to complete a self assessment form to establish what help you think you need.

Once you have completed the form, a care manager will go through the form with you to discuss the support you have identified.

We have lots of organisations that now provide support to help you with your independence. You are no longer confined to the traditional services provided by the council.

Paying for services

Whilst advice and support with the assessment is free, most of the services the council provide will have to be paid for. How much you pay will depend on how much you can afford and a financial assessment will be carried out.

To find out more, visit our paying for your own social care – self-funding page, under related links.
