Complete all relevant sections of the form and return to us with the relevant supporting documents.
Copies of all certificates of qualification held by the applicant and/or their employees.
Two passport-sized photographs of each of all the individuals who will be carrying out the treatments with their names clearly written at the back of each photo.
Valid Electrical Certificates for the premises, certified by an electrical contractor whose name is on the roll of the National Inspection Council for Electrical Installation Contracting (N.I.C.E.I.C)
The correct application fee.
A copy of your Business Insurance Certificate.
A fee and charges list for the current year is provided with the application form, which should be used to calculate the correct fee.

What happens to my application once you receive it?
When we receive your completed application form we carry out a consultation for a period of 28 days. We will send a photocopy of the application to each of the following asking them for any observations they may have towards the grant of a Special Treatments Licence to the applicant:

The applicant and premises will then be subject to an inspection by officers from the health and safety team and by the local fire and police authority (as deemed appropriate).
A public notice is also sent to the applicant to display on the window or door of the premises, for the public to easily view for a period of 28 days, which allows them to make any observations they may have towards the application.
If the fire, police and safety officers are satisfied with the premises the licence is issued with conditions attached and the applicant can trade.
