Children in and after care

Pathways works with 16 to 21 year olds (up to 24 if in higher education) who are or have been in care in Hounslow under the Children Act 1989 or the Children (leaving care) Act 2000.

We work with children and young people born in the UK and unaccompanied asylum seeking children. Please see links on this page for further information on asylum seeking unaccompanied children.

Each young person receives a needs assessment and plan, which records the young person's aspirations and identifies the goals they wish to achieve.  This is regularly reviewed.

We aim to make sure a positive transition for young people into adulthood, and offer support, advice and guidance on a range of issues affecting young people.

We have specialist knowledge and can advise on:

  • benefits

  • welfare rights

  • accommodation

  • health

  • sexuality

  • many other issues

We also work closely with other departments and organisations in order to access specialist services, for example, housing, Connexions, education, health.

We are open:

  • Monday, Tuesday and Thursday 9am to 5pm

  • Wednesday 1pm to 5pm

  • Friday 9am - 4.45pm
