The social care advisors in the adult access team are trained to help with issues or requests for information relating to your social care needs.
The team provide a confidential information and assessment service, whether you are enquiring about help at home following loss of independence, day care for someone or a carer’s assessment for yourself.
To understand what help you need, the social care advisor will ask you a number of questions about your health, your family circumstances, what you can and cannot do at home and what help you think you may need. The information you provide will help them to decide whether you may be eligible for services.
If you are not eligible, they will provide you with information about other services that may be able to help.
If they think you may be eligible, the information you provide will be given to our specialist assessment team, who will contact you and arrange to visit you to carry out an assessment of need at home.
